Bornemann Skilled Nursing Home, Inc. is certified for Medicare A and B, medical assistance and private pay. We are monitored by Wisconsin Bureau of Quality Assurance teams (registered nurses, social workers and engineers) every nine to 15 months. Twenty-four hour care is provided by R.N.s, L.P.N.s, nursing assistants, social workers, trained activity staff and food service workers. Rehabilitation is provided by physical, occupational and speech therapists. All medical care is provided under specific medical standards and directed by each resident’s personal physician.
