Bornemann Skilled Nursing Home, Inc. is certified for Medicare A and B, medical assistance and private pay. We are monitored by Wisconsin Bureau of Quality Assurance teams (registered nurses, social workers and engineers) every nine to 15 months. Twenty-four hour care is provided by R.N.s, L.P.N.s, nursing assistants, social workers, trained activity staff and food service workers. Rehabilitation is provided by physical, occupational and speech therapists. All medical care is provided under specific medical standards and directed by each resident’s personal physician.

Bornemann CBRFs are licensed by Wisconsin in each county under state statute Wisconsin D.H.S. 83 which is less stringent than that of nursing homes. There are levels of licensure pertaining to the amount of care given residents at each facility. The staff receives training in fire safety, first aid and choking, medication supervision, residents’ rights, client-specific needs and challenging behaviors. The licensing specialist monitors every two years or as necessary. An R.N. consultant is required and is on duty at Bornemann C.B.R.F. North and South.

The right accommodations for a patient’s medical, personal and physical care needs should be decided upon with help from your physician and family. Memory ability and social preferences also help with determining what would best benefit your loved one. You will also need to assess personal finances. Bornemann Skilled Nursing Home, Inc. is staffed above standards required by Wisconsin, ensuring proper care and timely response to our residents’ needs. Each CBRF determines staff based upon the residents it serves and level of licensure.

Medicare – Medicare does not cover personal care such as assistance with bathing, dressing, and eating. It will not cover care in an assisted living facility.
Medicaid – In some states Medicaid may help pay for personal care and housekeeping services in an assisted living facility, but only under the following conditions:

Medicaid requirements vary widely by state. If you know that you qualify for Medicaid or if you have specific questions about Medicaid, contact your local Medicaid office through your state.
Private pay – Many people pay privately for their care using personal resources. Over time they may become eligible for Medicaid if assets are reduced and meet program requirements.
Long-term care insurance – is a policy purchased by an individual. It is very important to understand exactly what kind of care the policy will cover, who will decide when the policy will take effect and the amount of dollar coverage per day needed.
